Controls in IT Development environments

There is an argument that if changes to a critical production application require development work then some assurance is needed on availability (and even continuity) of the IT development environment used to develop that application.

Has anyone come across this scenario?  Beyond control of source code (a must for any development) what controls did you apply to provide such assurance?

1. availability of source code -- do you keep your source code in just one place? if so is it a single point of failure? If this "place" goes down, would you be able to support your application? 2. Test environment -- where do you test your applications? If this test environment is unavailable would you still be able to support your application in production?
