December 2008 to June 2009 Exam Deferral Requests
and December 2008 Exam Cancellation Requests
Candidates may defer a fully paid December 2008 exam registration to the June 2009 exam date by making an exam deferral payment and completing the deferral request. If you wish to cancel your registration see the Refund Policy below.

A deferral request received on or before 17 October 2008** will be charged a US $50 processing fee.

A deferral request received from 18 October through 26 November 2008** requires a US $100.00 processing fee.

Deferral requests will not be accepted after 26 November 2008, 5:00 p.m. Central Time, Chicago, Illinois USA.
- Exam and deferral fees are non-refundable.

Requests received after 26 November 2008 and candidates who do not appear at their designated exam site by the required time will not be able to receive exam deferrals or refunds of the exam fees.

After your deferral is processed and the June 2009 exam is open for registration (December 2008), you will automatically be registered for that exam and receive an email and postal notification. By electing to defer your exam date, you agree to be bound by the terms and conditions cited below, which are consistent with the terms you agreed to during your previous registration process.
Deferral Request Options:
1) Online—available for credit card, wire or check - at the conclusion of the purchase process you will provide your credit card details or indicate that you will follow-up with sending a wire or check.
Please note: your deferral request will not be processed until deferral fees have been paid in full. Payment is due in full by 13 December 2008. Deferral fees are non refundable.

2) Faxable Form—for credit card charges only. A downloadable form will enable you to request the deferral and pay by credit card. All wire and check purchases must be performed using the online option.

By making an exam deferral request you agree to the following:
I hereby apply to ISACA for Exam by the Association in the form of the Certified Information Systems Auditor (CISA) and/or Certified Information Security Manager (CISM) and/or Certified in the Governance of Enterprise IT (CGEIT). By registering to take the Exam, I certify that I have read and agree to the conditions set forth in the Bulletin of Information covering administration of the exam; certification rules, policies and procedures; and the release of my test results; and I agree to disqualification from the Exam and/or nullification of any Exam score in the event that any statement or information provided by me to the Association is false or fails to include a material fact, or in the event that I violate any of the rules, policies or procedures governing the Exam. By applying for membership in the Association, I certify that I will abide by the Association's Code of Professional Ethics.
I hereby agree to hold the Association, its officers, directors, examiners, members, employees and agents harmless from any complaint, claim or damage arising out of (1) any action or failure to act by me on behalf of the Association, and (2) any action or omission in connection with my registration to take the CISA and/or CISM Exam, any Exam given by the Association, and any grade relating thereto and/or my application for membership. I understand that the final decision as to whether I pass the CISA and/or CISM exam and/or am accepted as a member of the Association rests solely with the Association. I further understand that ISACA may inform the local ISACA chapter and other appropriate parties of my having passed the exam. Notwithstanding the above, I understand and agree that any action arising out of or pertaining to this application or the CISA and/or CISM Exam must be brought in the Circuit Court of Cook County, Illinois, USA, and shall be governed by the laws of the State of Illinois, USA.

Refund Policy
Candidates unable to take the exam are eligible for a refund of registration fees, less a US $100 processing fee, if such a request is received in writing on or before 11 October 2008, 5:00 p.m. Central Time, Chicago, Illinois USA.
All requests for a refund after this date will be denied. A deferred registration is not eligible for a refund. Exam registration and membership fees are nontransferable.
